How does Lean confirm that the payment has been successfully made?

Once the payment has been successfully initiated Lean sends a webhook with the ACCEPTED_BY_BANK status. This status indicates that the bank is attempting to process the transaction - however, it is not a confirmation that funds are guaranteed to be received. 

In the meantime funds should be reconciled by you before making them available for use by your customer(s).
